Applicants

All applicants should complete a Self-Declaration Form. See Section 2. The form is intended to supply relevant information required for a position that involves working with young people that will:

  • Confirm their full name, current and recent addresses and date of birth;
  • Provide details of previous experience, voluntary or paid, of working with young people.
  • Gain permission to contact and provide details of at least two individuals (not relatives) who may be asked to provide a written reference.
  • Details of any convictions for criminal offences against children, including any 'spent' convictions.
  • Gain permission to carry out a criminal records disclosure.
  • The applicant should be provided with a copy of the organisation's Code of Conduct. .
It should also be stated on the form that failure to disclose information, or if appointed, that failure to conform to the Code of Conduct would result in disciplinary action and possible exclusion from the activities of BYAFA.
