Temporary membership forms are issued for insurance purposes to existing BYAFA member teams only. Over the recent past some clubs who are unregistered with BYAFA have been deliberately and wilfully misleading their players by adding a link to the documents page to encourage them to download the forms. It is quite possible that unwittingly they may believe they have adequate insurance cover. This is NOT the case.

Any individual or club official must understand that the use of these forms are for three sessions only and that any club or member of a club not listed on the Teams section of this site is not eligible for temporary insurance coverage.
